The mask is the defining visual detail. In the current build his face is never shown, and players have read that as a deliberate choice: the Doctor is defined by his function (healing) rather than his identity. Whether the mask hides a connection to an existing character or is simply costume design is not confirmed by the developer.
The Darthsuki Connection
The Doctor is voiced by Darthsuki - the same voice actor who plays Pierrot. This is confirmed on the official itch.io page and is one of the reasons the community has paid close attention to him: shared voice casting is rarely accidental in a small indie project, and players have speculated that the link is thematic rather than logistical.

The most common reading is that the shared voice ties the Doctor to Pierrot thematically - both are figures of care in a world built on consumption, even if they express it very differently. Pierrot's care is possessive and yandere-coded; the Doctor's is clinical and masked. Whether the developer intends anything deeper is not confirmed.
